Using this form

Use this form to apply for a National Entitlement Card (NEC) or a Young Scot NEC, with free bus travel, on behalf of a child aged under 16 years.

To use this form, your child/children must attend school in the Scottish Borders, and you must be resident in the Scottish Borders area. You can apply for a card for more than one child, however you must all live at the same address. As the person completing this form, you should be the child's main contact.

If any of the children you are applying for are over the age of 11, you should have the following evidence ready to upload before submitting the application:
