1: If any service users with care needs assessed as low or moderate risk are receiving care packages to mitigate those needs 1(a): If the answer is yes, the average annual value of care packages for the mitigation of these specific needs 2: Of the individuals receiving a care package within the Physical Disabilities and Long Term Conditions team caseload, the current proportion of people choosing Self Directed Support for each of the four SDS options
3: The current percentage of individuals receiving a care package within the Physical Disabilities and Long Term Conditions team caseload, who receive any form of respite provision within their care package (note, this does not include respite funded through an Adult Carer Support Plan for the carer rather than the service user) 3(a): Within the population of individuals receiving such respite provision, please provide the percentage breakdown of the type of provision provided (e.g. Respite Nursing Home Care, Respite Residential Home Care, Day Service) along with a definition of how the council defines those provisions. This percentage breakdown should also include the percentage of people receiving a direct payment to organise their own services, commonly known as In-Lieu Day Service - Response
- SBC apply an agreed eligibility criteria which is set at critical and substantial level (based on the national Fair Access to Care EC). We do not meet low or moderate needs as assessed under this criterion. If people do not meet our eligibility criteria for support, we assist them to access third sector or other non-statutory community-based services. Some people however may have higher needs and lower risks and so it is the overall category of need that indicates which support is required. See attached See attached SBC offer a range of respite in the form of building based stays in residential or nursing homes, plus flexibly funded respite to cover short stays of support at home or away, or a small number of support hours per week to be used flexibly. SBC day services to include a range of day and social support opportunities from one-to-one community based support, through to support to attend lunch clubs and buildings based day services.
