Request or Question
Under the Freedom of Information Act, and with relation to the below site or sites operated by Asda Stores Limited (or other companies within the Asda group), please can you provide me with details of: 1) The number of RIDDOR reports that have been made regarding incidents at each site, broken down by the financial years 2019/20, 2020/21, 2021/22, 2022/23, and 2023/24;
2) The cause of each such incident, broken down by broad heading, where this information is held;
3) Copies of any health and safety inspection reports held by the authority that arose from inspections of the below site or sites during the same period;
4) Copies of any reports arising from health and safety investigations held by the authority that took place in response to incidents at the below site or sites during the same period; and
5) Details of complaints made about the site or sites by members of the public during the same period, including the numbers and reasons for such complaints, and copies of the complaints where they are held in a reasonably accessible and anonymised format. The site or sites in question are: ASDA - GALASHIELS STORE 5876 Galashiels Currie Road, Galashiels, Scotland TD1 2AG If the address/addresses are identifiable in your system with minor variations to the address given here then I would be grateful if the response could be returned for the relevant address as you hold it.
Response
1) One RIDDOR report in year 2021/22, no other RIDDOR reports during the periods requested.
2) The cause of the incident was a collision between a member of staff and trolley, being pushed by another member of staff, causing pain in their elbow and them being off work for over 7 days. No investigation was carried out by SBC.
3) No H and S reports were produced during the periods requested.
4) There was an incident on 17th December 2022 when an employee called the police stating he d fallen into a compactor in the warehouse and couldn t get out, nor could he raise his colleagues within the store. Police attended and he was found in the compactor, uninjured. The site was visited and an investigation carried out but no report was produced. A note was made on the Uniform record Visited and spoke to store manager. Accident seemed to be employees fault going against demonstrated training. Rang police by mistake and was rescued unharmed. Saw risk assessments and investigation and happy with the store response. Further training to be completed by all staff and keys to the compactor kept restricted to managers. Compactor is not inherently dangerous if used properly, interlock is working and 'escape hatch' is actually a maintenance access that should be kept locked. NFA.
5) 4.11.20 complaint re number of customers not adhering to COVID control measures, masks and social distancing, referred to Police 15.11.21 complaint re out of date food delivered to customer for the site customer advised to contact store for refund 13.1.23 complaint that frozen chips in display freezer looked like they d been defrosted and refrozen manager contacted, no issues noted with telemetry NFA
