Downloading the forms

To download a form in this section you should save the document to your desktop, otherwise it may open in your browser window. Note it may take a few minutes to download the larger documents.

All forms in this section MUST be printed out, filled in and sent back to us or handed in to us in person at your local contact centre. They CANNOT be saved or emailed to us. Please remember to sign the forms at the end!

Download the Housing Benefit and Council Tax Benefit form (pdf, size 524Kb). To save this from a PC simply right click your mouse and save target as your desktop. You can then print out the form and fill it in.
Download the Self Employed Earnings form if you answered YES to Part 6 of the main Housing Benefit and Council Tax Benefit form.

Returning Benefit Forms

So we can deal with your claim quickly please bring your completed form (and any supporting documentation) to your local contact centre. This will allow us to get through your form with you in person and check the details. If you cannot get to your local contact centre to hand in your form, please send it to:
