Summary
Download our claim form, complete it and hand it in to your local contact centre along with the supporting information needed (details of the information required are explained in the claim form).
If you cannot provide the supporting information we need at the moment, you should still return the form NOW, and give us the supporting information later.
We can start to assess your claim but will not be able to pay you any benefit until we have all the information we need.
Occasionally PDF Files do not display correctly.
At some point between downloading the file, your browser opening Adobe Reader, and then displaying the file - it has become
corrupted.
Trying to re-open the PDF does not seem to help.
The following suggestions should help:
- clear your cache (find out how)
or - right click on the file, choose 'save target as' and save the pdf to your desktop. Double click the file to open it
If you are still having problems please contact us and include
- the name of the file
- the link (url) to the file
It also helps to make sure that you have the lastest versions of:
- adobe reader
- your browser software (Safari, FireFox or Internet Explorer)
