Application for affirmation of provisional grant of licence

Summary
An outline provisional grant made under Section 26(2) becomes “ineffective” unless “affirmed” within 12 months. The application for affirmation must be accompanied by detailed plans of the premises. To apply for a licence on the application form can be downloaded, completed and returned with the fee to Licensing Section.
Occasionally PDF Files do not display correctly.
At some point between downloading the file, your browser opening Adobe Reader, and then displaying the file - it has become
corrupted.
Trying to re-open the PDF does not seem to help.
The following suggestions should help:
- clear your cache (find out how)
or - right click on the file, choose 'save target as' and save the pdf to your desktop. Double click the file to open it
If you are still having problems please contact us and include
- the name of the file
- the link (url) to the file
It also helps to make sure that you have the lastest versions of:
- adobe reader
- your browser software (Safari, FireFox or Internet Explorer)
