How to apply
You will have to fill in a form which you can get and return to any contact centre or library.
Return your completed form along with:
- passport style photograph
- proof of your age
- proof of your address

The following documents can be used:
- attendance allowance award letter
- birth certificate
- passport
- photo driving licence
Proof of address
The following documents can be used:
- bank statement dated within the past three months
- Council Tax bill for the current year
- current television licence
- gas, electricity or landline phone bill dated within the past three months
- letter from residential home
